I hereby give _______________________________________________ (legal name of person submitting video), their legal representatives, and those acting with its authority or permission, the absolute right and permission to use and distribute solely my image and voice, to the 1st Annual Los Angeles Fetish Film Festival.
Entries submitted to the competition WILL NOT be returned.
I also grant the use of my image which may be used in whole or in part, or composite, or distorted in character or form in conjunction with my own fictitious name or real name, made through this medium (videotape format). I hereby waive any right that I may have in inspecting and/or approving the finished product that may be used in connection therewith or the use to which it may be applied.

(legal name of person submitting video), Los Angeles Fetish Film Festival/American Fetish Film, LLC, its staff and business associates, any and all persons involved with the creation of this video show, their legal representatives or assigns, and all persons acting under their permission or authority of those for whom it s acting, from any liability by virtue of any blurring, whether intentional or otherwise, that may occur or be produced in taking of said picture or in any processing tending toward the completion of the finished product, even though it may subject me to ridicule, scandal, reproach, scorn, and indignity.
I expressly stipulate that the releases may utilize the released subject matter or not as they choose in their sole discretion without affecting the validity of this release.

I hereby warrant that I am over the age of 18 and have every right to contract in my own name in the above regard. I further state that I have read the above authorization, release and agreement, or it has been read to me, prior to its execution and that I am fully familiar with the contents thereof. I also understand that the creation of this video was done under my own free will and was not coerced against my will or desire.
