LAFetFilmFest: CHECKLIST

Checklist
The following documents must accompany every Entry:

1.  A completed Entry Form with the information as requested.

2.  A completed and signed copy of the “Records Keeping Compliance Form Pursuant to 18 U.S.C. §2257” for each Actor/Model appearing in the entry.  Every individual, including Extras, must have a 2257 form filled out on that specific individual.  The Entrant must fill out a form, too.

3.  Copies of all identification documentation required by the Records Keeping Compliance Form for each Model/Actor/Extra and for the Entrant.

4.  A completed and signed copy of the “Los Angeles Fetish Film Festival Agreement and Release” for each model/actor/extra in the submission.

5.  Fees & Deadlines:

Submissions after Sept. 25, 2011:            Late entry fee is $60 dollars.

6.  You may enter as many times as you like.  Each entry must have it’s own separate set of forms and entry fee.  No acceptions.

All the referred to documents must be printed and filled out.
